Holiday Programme Terms and Conditions
Medical Information:
The School Holiday Programme activities cater for all children including those with disabilities. Please provide medical conditions/allergies. Medication (with instructions on use) must be provided with child(ren) at all times.
Parent/Caregiver Permissions:
Please inform us if you do not give permissions for Stratford District Council Staff to administer first aid and seek treatment if necessary to assist us with the care and safety of your child/children
Please inform us if you do not give permission for yourself/child to be photographed and used in promotion of Wai O Rua.
Drop Off and Collection:
Children must be over 8years to be left alone and are not be left alone until the registration is confirmed and the activity commences.
Please advise us who you authorise to collect your child/children from our care other than yourself, children must be picked up within 15 minutes of the activity ending.
Please provide any special instructions regarding collection of your child/children
Additional Information
Please provide any other information that you feel would be beneficial to us in providing care for your child/children (e.g. dietary requirements)
Stratford District Council School Holiday Programme is run in accordance with Stratford District Council’s Child Protection Policy and aligns with the vulnerable children’s act 2014.
Payment must be made at time of registration.
